TRUCARE PAIN MANAGEMENT DOES NOT DISCRIMINATE IN ITS HEALTH PROGRAMS OR ACTIVITIES.
Notice
TruCare Pain Management does not discriminate on the basis of race, color, national origin, sex, age, or disability in its health programs or activities.
TruCare provides reasonable modifications for individuals with disabilities and appropriate auxiliary aids and services free of charge and in a timely manner when necessary to ensure equal access and effective communication.
TruCare provides language assistance services free of charge and in a timely manner when necessary to provide meaningful access for individuals with limited English proficiency.
These services may include:
- qualified interpreter services;
- translated written information when appropriate;
- information in alternate formats when appropriate;
- auxiliary aids or services for individuals with disabilities;
- reasonable modifications when necessary to support access to care.

Complaints
If you believe TruCare Pain Management has failed to provide these services or discriminated on the basis of race, color, national origin, sex, age, or disability, you may file a complaint with TruCare.
To request help or file a complaint with TruCare, contact the Office Manager by phone, fax, mail, or in person using the contact information above.
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
